Creates a statewide electronic system allowing individuals to upload and access advance medical directives including medical orders for scope of treatment, declarations on medical treatment, cardiopulmonary resuscitation directives, and medical durable powers of attorney.

Authorizes the Department of Public Health and Environment to contract with health information organization networks for creation and maintenance of the system, and to promulgate rules establishing access criteria, procedures for adding/removing directives, and confidentiality safeguards.

Requires qualified providers to upload directives only after in-person or telehealth consultation with the individual or authorized surrogate, and requires electronic affidavit signature affirming the directive is properly executed, current, and accurate.

Provides liability protection for qualified providers uploading directives, emergency medical personnel, healthcare providers, and facilities complying with directives accessed from the system unless they have actual knowledge of a properly executed later directive.

Appropriates $993,147 to the Department of Public Health and Environment for fiscal year 2019-20, including $750,000 for information technology services, and $750,000 to the Office of Information Technology for IT services.
