Creates a special "Hospice and Palliative Care" license plate available starting January 1, 2022 for motorcycles, passenger cars, trucks, and non-commercial motor vehicles under 16,000 pounds.

Applicants must make a donation to a designated Colorado nonprofit organization (40+ years old, holds annual statewide hospice/palliative care conference, provides education on these services) to qualify for the plate.

The designated organization may establish a minimum donation amount for initial issuance and require donations up to $100 for annual renewal; all donations must support hospice and palliative care programming in Colorado.

Special license plate holders pay regular motor vehicle registration fees plus a one-time $25 issuance fee; renewal donations up to $100 may be required by the designated organization.

Appropriates $6,907 for fiscal year 2021-22 to the Department of Revenue ($5,400 from highway users tax fund and $1,507 from license plate cash fund) to implement the program.
