Creates a "Support Pollinators" special license plate available for motorcycles, passenger cars, trucks, and noncommercial/recreational vehicles not exceeding 16,000 pounds empty weight.

Requires the Department of Revenue to designate a Colorado-based nonprofit organization at least every five years that has existed for 5+ years and works to promote native pollinator health, habitat, and public education.

Applicants must provide a donation certificate from the designated organization and pay standard license plate fees plus an additional one-time fee of $25, which is deposited into the Highway Users Tax Fund.

Allows personalized support pollinator plates with additional fees under existing personalization rules, and permits transferring existing personalized combinations to the new plates.

Appropriates $22,544 to the Department of Revenue for the 2021-22 fiscal year ($5,400 from the Colorado DRIVES vehicle services account and $17,144 from the license plate cash fund).
