Health-care providers performing induced terminations of pregnancies must report specified information to the state registrar of vital statistics, excluding any identifying information about the women.

Required reporting includes woman's age, race, ethnicity, marital status, education level, number of previous terminations, number of living children, gestational age, stated reason for procedure, impediments encountered, procedure cost and funding source, procedure type, and any complications.

State registrar must annually create and publicly release a summary report of the information but cannot release facility names or provider identities.

Physicians, physician assistants, and advanced practice registered nurses who falsify or fail to submit required information face discipline under the Colorado Medical Practice Act and Nurse and Nurse Aide Practice Act respectively.

Act takes effect 90 days after final adjournment of the general assembly, unless a referendum petition is filed, in which case it requires voter approval at the November 2022 general election.
