Creates a Special Olympics Colorado license plate called the "Teri Leiker Act," with issuance beginning on or before January 1, 2022, for motorcycles, passenger cars, trucks, and noncommercial vehicles not exceeding 16,000 pounds empty weight.

Designates an organization to qualify applicants that must be headquartered in Colorado, exist for at least 40 years, provide year-round sports training for children and adults with intellectual disabilities, and collaborate with schools on inclusive activities.

Requires applicants to donate between $60 and $100 to the designated organization and provide a certificate of donation to the Department of Motor Vehicles to qualify for the plate.

Imposes a one-time additional fee of $25 for issuance or replacement of the special license plate, with funds credited to the Highway Users Tax Fund; allows personalized versions with additional fees under existing personalized plate provisions.

Appropriates $13,460 to the Department of Revenue for fiscal year 2021-22, consisting of $5,400 from the Colorado DRIVES vehicle services account and $8,060 from the license plate cash fund for implementation.
