• Authorizes adults with disabilities to enter into voluntary supported decision-making agreements as an alternative to or supplement for guardianship, allowing them to retain decision-making authority while receiving assistance from a supportive community.

• Requires supported decision-making agreements to contain the adult's name, supporter contact information, list of decisions to be advised on, agreement terms, notice about mandatory reporters, and signature date, with signatures witnessed by two disinterested witnesses or a notary public.

• Establishes that agreements do not create fiduciary relationships, alter court-ordered guardianships or conservatorships, authorize access to personal information for supporters' own benefit, or shield supporters from criminal or civil liability for wrongdoing.

• Automatically terminates agreements if Adult Protective Services substantiates mistreatment, or if the supporter is convicted of crimes against at-risk persons, financial crimes, or theft.

• Provides liability protection for persons relying on supported decision-making agreements in good faith, and allows supporters to access personal information only when specifically requested and for purposes the adult directs.
