Health-care providers performing induced terminations of pregnancies must report specified information to the state registrar of vital statistics in electronic format, with reports based on complete reproductive health-care histories.

Reported information must include woman's age, race, ethnicity, marital status, prior terminations, living children, gestational age, contraception method used, and reasons for the procedure (including education/employment interference, financial insecurity, health concerns, and fetal abnormality).

Reports must not include any information that could identify the woman obtaining the procedure or the health facility where the procedure was performed.

State registrar must annually create a public summary report of aggregated information and may permit use for statistical or research purposes under specified conditions; disclosure to individual health facilities or providers is prohibited.

Physicians, physician assistants, and advanced practice registered nurses who falsify or fail to submit required information engage in unprofessional conduct subject to disciplinary action under Colorado Medical Practice Act and Nurse Practice Act.
