Establishes a Colorado Rotary license plate available on or before January 1, 2023 for motorcycles, passenger cars, trucks, and noncommercial vehicles not exceeding 16,000 pounds empty weight.

Requires applicants to provide a certificate from a Rotary district (5440, 5450, or 5470) confirming membership in good standing and pay standard vehicle license taxes and fees plus an additional one-time $25 fee.

Directs the Department of Revenue to use a design provided by Rotary districts that conforms to departmental standards, with the $25 additional fee credited to the Highway Users Tax Fund.

Allows qualified applicants to obtain personalized Rotary license plates by paying the standard personalized plate fee and may transfer existing personalized plate combinations to new Rotary plates.

Appropriates $18,184 for fiscal year 2022-23 to the Department of Revenue ($15,976 from general fund, $2,208 from license plate cash fund) to implement the new plate program, including system maintenance and information technology services.
