Defines "personal rights of a protected person" as the right to personal autonomy and constitutional rights including freedom of speech, religion, association, due process, and equal protection under law.

Requires guardians to notify within 7 days one or more close family members and designated persons when a protected person changes residence, resides away from home for more than 48 hours, is admitted for acute or emergency medical care, or dies; close family members must be notified in person or by phone while court record persons receive written notice.

Mandates guardians develop an initial care plan within 60 days of appointment in conjunction with the protected person and close family members, covering why guardianship is necessary, visitation restrictions, financial handling, and access to information; plan must be updated annually.

Requires guardians to notify the court and close family members within 7 days of any changes in the protected person's condition, custodial residence, or death; exceptions apply when a person requests no notice in writing or when court orders prohibit notification.

Exempts guardians from providing notice when an order of protection or restraining order has been issued against the person on behalf of the protected person.
