• Creates Community First Choice optional Medicaid benefit allowing state to seek federal authorization by July 1, 2025, for home- and community-based attendant services and supports

• Covers four main service types: personal care services, homemaker services, health maintenance activities, and electronic monitoring services, delivered through in-home support, consumer-directed, or licensed home care models

• Establishes eligibility requiring Medicaid enrollment and either membership in a nursing facility coverage group or income at or below 150% of federal poverty level, with annual determination that individual would require institutional care without services
