Requires health-care providers to disclose their name, employer, job title, academic credentials, and professional credentials to patients in writing and via photo identification badge beginning January 1, 2025.

Disclosure requirements apply to services or procedures requiring informed consent that are provided in licensed health-care facilities.

Photo identification badges must be sufficiently sized to include a picture of the provider and must be worn during all patient encounters unless precluded by isolation or sterilization protocols.

Willful or repeated failure to comply constitutes unprofessional conduct or grounds for discipline under the provider's licensing laws, but does not create a private right of action for patients.

Licensed or certified health-care facilities are required to ensure that providers comply with the identification and badge requirements.
