Authorizes the Colorado Healthcare Affordability and Sustainability Enterprise to charge and collect new provider fees on nursing facilities and intermediate care facilities for individuals with intellectual disabilities, beginning May 1, 2025

Creates two new cash funds: the Healthcare Affordability and Sustainability Nursing Facility Provider Fee Cash Fund and the Intermediate Care Facility Fee Cash Fund to receive collected fees and leverage federal matching funds

Establishes a Facility Provider Fee Enterprise Support Board (8 members appointed by the Governor) to advise on implementation of the nursing facility and intermediate care facility fees

Uses nursing facility provider fee revenue for supplemental Medicaid payments based on case-mix/acuity, quality performance measures, and care for residents with mental health conditions, dementia, or acquired brain injury

Transfers existing balances from the Medicaid Nursing Facility Cash Fund and the Service Fee Fund to the new enterprise-administered cash funds on June 30, 2025, and repeals the prior fee programs effective July 1, 2025
