Department of Social Services shall implement and expand a pilot program delivering health care services through primary care case management to at least 1,000 individuals eligible for HUSKY Plan, Part A benefits.

Primary care providers in the pilot program coordinate all health care services for beneficiaries, including primary care and arrangement of specialty care as needed.

Department shall expand the pilot program to include primary care providers in Torrington and Putnam by July 1, 2010 (Putnam) and October 1, 2010 (Torrington).

Commissioner of Social Services may seek a federal waiver to implement the expansion of the primary care case management system.

Commissioner shall report to the joint standing committees on human services, appropriations, and state agency budgets by July 1, 2011 regarding the pilot program expansion.
