Requires nursing home licensees and owners to submit a written change in ownership application to the Department of Public Health for facilities acquired after July 1, 2004.

Mandates disclosure of civil penalties imposed within two years, intermediate sanctions under Medicare or Medicaid, or termination or nonrenewal of provider agreements in any state.

Prohibits the commissioner from approving acquisition of another nursing home for five years from the date of final order on civil penalties, sanctions, or termination, except for good cause shown.

Requires the Department of Public Health to print a notice in at least 18-point boldface type on all change in ownership applications warning that nursing home licensees, owners, officers, directors, and key staff may face criminal liability for employee abuse or neglect of residents.

Directs the Department of Public Health to distribute the criminal liability notice to all existing nursing home licensees by January 1, 2011.
