Requires individual and group health insurance policies to cover baseline mammograms for women aged 35-39 and annual mammograms for women age 40 and older, effective January 1, 2011.

Mandates coverage for comprehensive ultrasound screening and magnetic resonance imaging of the breast when mammograms show dense or heterogeneous breast tissue, or when a woman is at increased risk for breast cancer based on family history, personal history, genetic testing, or physician assessment.

Requires all mammography reports to include information about breast density using the Breast Imaging Reporting and Data System and to provide a standardized notice about supplementary screening options when applicable, effective October 1, 2009.

Applies the same coverage requirements to both individual health insurance policies and group health insurance policies under the specified insurance categories.
