Allows adult adopted persons whose adoptions were finalized on or after October 1, 2011, to access their sealed original birth certificates and any attached contact preference forms, effective October 1, 2011.

Establishes a contact preference form system for birth parents to indicate whether they wish to be contacted by the adopted person or authorized applicants, with options for direct contact, contact through an intermediary, or no contact.

Permits birth parents to complete and submit health history forms that will be attached to the sealed original birth certificate for access by the adopted person or authorized applicants.

Maintains existing consent requirements for disclosure of identifying information about biological parents and relatives for adoptions finalized before October 1, 2011, with limited exceptions for cases where one parent consents and the other cannot be located.

Clarifies that access to original birth records under these provisions does not require a court order for eligible adopted persons, distinguishing this from other parties seeking access to adoption records.
