Limits patient out-of-pocket costs (coinsurance, copayments, deductibles) to no more than 20 percent of the testing cost per year, with an exception for high deductible health plans

Mandates testing be performed only at facilities accredited by the American Society for Histocompatibility and Immunogenetics and certified under the Clinical Laboratory Improvement Act of 1967

Requires individuals to complete and sign an informed consent form authorizing test results to be used for participation in the National Marrow Donor Program in order to receive coverage

Permits insurance policies to limit coverage to a lifetime maximum benefit of one testing per individual
