Expands provisional ballot eligibility from federal elections only to all state and municipal elections and primaries.

Requires moderators to appear at the registrars of voters office (not town clerk) the day before an election or primary to receive provisional ballot packets containing at least 1% of eligible voters' ballots or another mutually agreed-upon number.

Changes provisional ballot form to the regular ballot of candidates rather than a separate ballot, with the Secretary of the State prescribing the specific ballot format.

Requires registrars of voters to verify and count all provisional ballots within six days after the election or primary, prepare a report showing ballots received, rejected, and counted, and preserve the depository envelope for the same period as absentee ballots.

Repeals sections 9-232d to 9-232f and 9-232i of the general statutes and makes conforming changes to related election administration procedures and penalty provisions.
