Allows patients to request copies of health records or on-site examination of records from licensed health care institutions, including bills, laboratory reports, prescriptions, and retained tissue slides and blocks.

Limits charges for providing health records to 65 cents per page plus first-class postage, except for x-rays and retained tissue materials which may be charged at actual cost.

Requires institutions to furnish requested records or allow on-site examination within 30 days of request, or immediately upon completion if the request was made within 30 days of patient discharge.

Prohibits institutions from denying access to records based on inability to pay and requires those claiming inability to pay to submit an affidavit as presumptive evidence.

Establishes a complaint process allowing patients to file with the Department of Public Health and requires the Commissioner to conduct hearings within 30 days; if non-compliance is found, the commissioner must order disclosure within 5 days.
