Increases annual coverage for ostomy-related supplies from $1,000 to $5,000 for individual and group health insurance policies, including collection devices, irrigation equipment, skin barriers, and related items.

Requires health insurance policies to cover prosthetic devices (artificial limbs with microprocessors if medically necessary) at least equivalent to Medicare coverage, with medically necessary repairs and replacements covered unless due to misuse or loss.

Expands hearing aid coverage for children from age 12 to age 18 or younger, with coverage limited to $1,000 within a 24-month period.

Extends wig coverage for patients with hair loss due to chemotherapy or diagnosed alopecia areata (prescribed by licensed physicians or advance practice registered nurses) and requires coverage for human leukocyte antigen testing for bone marrow transplantation with out-of-pocket expenses capped at 20 percent.

Requires insurers to offer reasonably designed health behavior wellness programs with incentives (up to 20 percent of paid premiums) for participation, and eliminates the requirement for colonoscopies ordered by physicians in a policy year to be subject to copayments, coinsurance, or deductibles.
