Commissioner of Social Services shall amend the Medicaid state plan under Section 1915(i) of the Social Security Act to improve access to home and community-based services for elderly persons eligible under the state-funded home care program using needs-based eligibility criteria.

Commissioner may amend a previously submitted Section 1915(c) waiver application if necessary to implement the provisions.

Commissioner shall submit an application to participate in the State Balancing Incentive Payment Program under Section 10202 of the Patient Protection and Affordable Care Act to increase federal matching assistance payments for Medicaid home and community-based services.

Commissioner must establish a no wrong door-single entry point system for consumers to access information about available services and receive eligibility assessments.

Commissioner must implement conflict-free case management services to develop individual service plans and monitor services, and utilize a standardized core assessment instrument for eligibility determination.
