Requires individual and group health insurance policies to cover comprehensive breast ultrasound screening when a mammogram shows dense or heterogeneous breast tissue, or when a woman is at increased risk for breast cancer based on family history, prior personal history, genetic testing, or physician determination.

Eliminates all out-of-pocket costs (coinsurance, copayment, deductible) for breast ultrasound screenings, except for high deductible health plans.

Requires all mammography reports provided to patients to include information about breast density using the Breast Imaging Reporting and Data System and a notice about potential benefits of supplementary screening tests.

Maintains existing requirements for baseline mammograms for women ages 35-39 and annual mammograms for women age 40 and older.

Effective date: January 1, 2012.
