Establishes permanent supportive housing initiatives (replacing the previous two-phase Supportive Housing Initiative structure) to provide affordable housing and support services to individuals and families with special needs and those at risk for homelessness.

Expands collaboration to include the Departments of Correction and Mental Health and Addiction Services, the Judicial Branch's Court Support Services Division, and other agencies in developing and implementing the initiatives.

Directs the Departments of Mental Health and Addiction Services and Social Services to issue requests for proposals for scattered site housing models serving homeless individuals with psychiatric disabilities and substance use disorders.

Reserves $2 million annually in state tax credits (until 60 days after the Connecticut Housing Finance Authority publishes its housing program list) for permanent supportive housing initiatives and $1 million for workforce housing.

Authorizes state bond assistance up to $105 million through the Connecticut Housing Finance Authority for mortgage loans and related costs under the permanent supportive housing initiatives.
