Prohibits minors (persons under 18 years of age) from using tanning devices at commercial tanning facilities without prior written consent from a parent or guardian who is physically present.

Requires tanning facility operators to provide written materials to both the minor and accompanying parent or guardian detailing health risks associated with tanning devices, including the risk of developing melanoma, before the minor's first use.

Establishes a fine of up to $100 for operators who fail to comply with consent and health information requirements, with fines payable to the municipal health department or health district where the facility is located.

Authorizes municipal health departments and district health departments to enforce the provisions of this section within their available resources.

Takes effect on October 1, 2011.
