Requires insurers, health care centers, managed care organizations, and preferred provider networks to maintain provider networks meeting National Committee for Quality Assurance (NCQA) or URAC standards effective January 1, 2013.

Mandates that health insurance identification cards issued to enrollees include the name of the enrollee's primary care provider.

Requires managed care organizations to provide enrollees annually with a listing of all available providers, with advanced practice registered nurses listed in a separate category.

Allows enrollees in managed care plans requiring primary care provider selection to designate either a participating physician or a participating advanced practice registered nurse as their primary care provider.

Requires managed care organizations to notify enrollees as soon as possible when their primary care provider terminates or withdraws from the plan.
