Prohibits automobile physical damage appraisers and insurance companies from requiring that repairs or automotive glass work be performed at specified facilities or repair shops.

Prevents insurance companies from requiring insured individuals to use specific providers for motor vehicle physical damage repairs or glass replacement services, or from stating that choosing non-participating shops will cause delays or void guarantees.

Requires appraisals and estimates for motor vehicle physical damage claims to include a notice in at least ten-point boldface type stating: "YOU HAVE THE RIGHT TO CHOOSE THE LICENSED REPAIR SHOP WHERE THE DAMAGE TO YOUR MOTOR VEHICLE WILL BE REPAIRED."

Mandates that insurance companies and third-party claims administrators inform insureds during initial contact about glass claims that they have the right to choose their glass shop, and prohibits directing insureds to affiliated glass shops without providing names of additional shops in the repair area.

Amends insurance identification cards for private passenger motor vehicles to include notice that insureds have the right to choose the licensed repair shop or glass shop where damage will be repaired, effective October 1, 2012.
