Establishes that temporary state permit applications must be submitted on a single application form prescribed by the Commissioner of Emergency Services and Public Protection, which cannot be modified or supplemented with additional forms.

Requires applicants to provide: a completed notarized application, proof of lawful U.S. presence (birth certificate, naturalization certificate, valid U.S. passport for citizens; permanent resident card for aliens), a safety course certificate signed by an NRA-certified instructor or approved agency, and two sets of fingerprints.

Local authorities must inform applicants of approval or denial within eight weeks of receiving a sufficient application and forward a copy to the Commissioner.

Commissioner must approve or deny applications within eight weeks of receiving local authority approval, or notify applicants that national criminal history records check results are pending.

Takes effect October 1, 2012 and amends Section 29-28a of the general statutes.
