Requires local and regional boards of education, institutions, and facilities providing special education to record each instance of physical restraint or seclusion used on children and specify whether it was part of an individualized education program or an emergency.

Mandates the State Board of Education to review annual compilations from schools and facilities and produce an annual summary report on restraint and seclusion use, submitted by February 15, 2013, and December 15 annually thereafter.

Exempts in-school suspensions as defined in section 10-233a from reporting requirements.

Requires schools and facilities to report incidents of physical restraint or seclusion resulting in physical injury to the State Board of Education, which must include such incidents in its annual report.

Directs the State Board of Education and relevant agency commissioners to report any serious injuries or deaths to the Office of Protection and Advocacy for Persons with Disabilities and the Office of Child Advocate.
