Requires health care providers to submit third-party claims on standard HCFA 1500 or UB-92 health insurance claim forms, or other forms prescribed by the Insurance Commissioner, effective October 1, 2012.

Permits health care providers to use a National Provider Identifier (NPI) assigned by the Centers for Medicare and Medicaid Services in place of a federal tax identification number on claim forms.

Establishes specific required information fields on the HCFA 1500 form that, when completed, prevent claims from being deemed deficient for processing purposes, including patient identification, dates of service, diagnosis codes, and charges.

Directs the Commissioner of Public Health, in consultation with the Insurance Commissioner, Commissioner of Mental Health and Addiction Services, Attorney General, and Healthcare Advocate, to develop a uniform treatment authorization form for mental health services by January 1, 2013.

Requires the uniform mental health authorization form to comply with state and federal privacy standards under HIPAA and federal mental health parity laws, with proposed implementing legislation due to the joint standing committee by January 15, 2013.
