Commissioner of Social Services shall seek to amend the Medicaid state plan under Section 1915(i) of the Social Security Act to improve access to home and community-based services for elderly persons eligible for the state-funded home care program using needs-based eligibility criteria.

Commissioner may amend previously submitted waiver applications under Section 1915(c) of the Social Security Act as necessary to implement the bill's provisions.

Commissioner shall submit an application to participate in the State Balancing Incentive Payment Program under the Patient Protection and Affordable Care Act to increase federal matching payments for Medicaid home and community-based services.

Implementation requires establishing a "no wrong door" single entry point system for service information and eligibility assessments, providing conflict-free case management for service planning and monitoring, and utilizing a standardized core assessment instrument for determining eligibility and appropriate services.
