Amends the deadline for the Commissioner of Consumer Protection to prescribe the residential property condition disclosure report form from April 1, 2010 to January 1, 2013

Requires the disclosure form to include information about municipal assessments (sewer or water charges), including current status, unpaid amounts, and potential future assessments

Mandates disclosure of leased items on the premises such as propane fuel tanks, water heaters, major appliances, and alarm systems

Requires disclosure of whether the property is located in a village district, historic district, or on the National Register of Historic Places, with information sources identified

Adds new requirement to disclose whether the property is subject to or has been subjected to remediation of hazardous waste as defined in section 22a-115
