Increases the maximum age limit for health insurance coverage of infertility treatment from 40 to 45 years old for individuals with individual health insurance policies.

Increases the maximum age limit for health insurance coverage of infertility treatment from 40 to 45 years old for individuals with group health insurance policies.

Maintains existing coverage limitations including four cycles of ovulation induction, three cycles of intrauterine insemination, and two cycles of in-vitro fertilization with up to two embryo implantations per cycle.

Preserves requirements that covered treatments be performed at facilities meeting American Society of Reproductive Medicine or Society of Reproductive Endocrinology and Infertility standards.

Takes effect January 1, 2013.
