Requires public and nonpublic schools to notify the Departments of Education and Public Health when a child is enrolled without receiving all vaccines required by the state's active immunization schedule and is not exempt from the requirement.

Maintains existing immunization requirements for diphtheria, pertussis, tetanus, poliomyelitis, measles, mumps, rubella, and hemophilus influenzae type B before school enrollment.

Preserves existing exemptions including medical contraindications, religious beliefs, previous disease confirmation, and age-based exceptions.

Takes effect October 1, 2012.
