Directs the Commissioner of Social Services to develop and administer a Community Choices program serving as the state's Aging and Disability Resource Center in accordance with the federal Older Americans Act Amendments of 2006.

Establishes eligibility for individuals age 60 and older, individuals age 18 and older with disabilities, and caregivers.

Requires the program to provide information and referral services, comprehensive assessments, counseling on employment and public benefits, follow-up assistance, care coordination, and a toll-free telephone number.

Authorizes the commissioner to establish program requirements, procedures for operating agreements, and adopt regulations necessary to implement the program.

Takes effect upon passage.
