Creates a Community Choices program administered by the Commissioner of Social Services to provide coordinated information and access to long-term support services for individuals aged 60+, those 18+ with disabilities, and caregivers, serving as Connecticut's Aging and Disability Resource Center Program.

Replaces the existing Medicaid waiver model with a new Section 1915(c) Medicaid waiver program to serve individuals aged 21 and under with physical disabilities (and possible co-occurring developmental disabilities) who are institutionalized or at risk of institutionalization and meet financial eligibility criteria.

Modifies the municipal agent for elderly persons role to make duties discretionary rather than mandatory, allowing agents to be employees of aging service agencies or elected officials rather than requiring commission on aging membership, and streamlines reporting requirements to focus on the Department of Social Services.

Expands Medicaid third-party liability requirements to include third-party administrators and requires health insurers to provide coverage information within 90 days of request and monthly thereafter, allowing automated data matching to identify insurance coverage for Medicaid recipients.

Repeals three statutes (17b-221a, 17b-342a, and 17b-245a) and makes minor technical corrections including updating mandated reporter definitions and correcting grammatical references throughout social services regulations.
