Adult adopted persons age 21 and older (or authorized applicants if deceased) may request and receive sealed original birth certificates and contact preference forms attached to them, effective June 1, 2013.

Birth parents may file contact preference forms indicating whether they wish to be contacted, wish contact only through an intermediary, or do not want contact, which are attached to the adopted person's sealed original certificate.

Birth parents may submit health history forms that are attached to sealed original certificates and made available to adopted persons and authorized applicants.

Department shall maintain and publish quarterly statistics on original birth certificates released, contact preference forms filed, and birth parent contact preferences.

Modifications to confidentiality provisions in adoption law to accommodate adult access to original birth records while maintaining privacy protections for biological parents who do not consent to contact.
