Directs the Commissioner of Social Services to develop and implement a pilot program delivering social services in urban cities with 60,000 or more residents, effective July 1, 2012.

Authorizes grants to public or private entities to support program development and allows the commissioner to work with interested municipalities to establish delivery models for social services.

Requires the pilot program to emphasize door-to-door outreach, neighborhood center-based services, school-based services, and agency-based casework while serving as a model for transferring state funding and authority to municipalities.

Mandates the commissioner apply for necessary federal waivers and submit waiver applications to the legislature for review before filing with the federal government.

Establishes a two-year pilot program (July 1, 2012 to July 1, 2014) with a requirement to submit a report within 90 days of the first year detailing cost efficiency and recommending whether to permanently implement the community-based model.
