Establishes a pilot program in five school districts by January 1, 2014 for school-based health care centers, with districts selected by July 1, 2013 by the Commissioner of Public Health in consultation with the Commissioner of Education.

Pilot program may include extended hours of operation, services to students outside the host school district, behavioral health services, community outreach, and reimbursement from private insurance.

Requires the Commissioner of Public Health to submit annual evaluation reports by July 15, 2014 and each year thereafter to joint standing committees overseeing public health, education, children and appropriations.

Terminates the pilot program no later than January 1, 2017.
