Requires health care providers to notify patients at the time they receive services that their health care information may be included in the all-payer claims database program under section 19a-724 of the general statutes

Allows patients an opportunity to refuse the use of their health care information for inclusion in the all-payer claims database

Amends title 19a of the general statutes to implement these notification and opt-out requirements

Aims to protect patient privacy by giving patients control over whether their health care information is reported to the all-payer claims database program
