Amends section 19a-550 of the general statutes to require nursing home facilities and chronic disease hospitals to provide patients and families with information about patient safety incidents at the facility.

Requires disclosure of the number of patient safety incidents occurring at the facility during the two calendar years before the patient's admission.

Requires disclosure of corrective actions taken in response to safety incidents at the facility.

Requires facilities to provide information on how to access Department of Public Health records concerning safety violations at the facility.

Aims to ensure families have adequate information about care quality when placing a patient in a nursing home facility.
