Amends section 19a-528a to require potential nursing home licensees and owners to submit change of ownership applications prescribed by the Commissioner of Public Health, effective October 1, 2013.

Mandates the change of ownership application include an 18-point boldface notice stating that nursing home licensees, owners, officers, and certain administrators may face criminal, civil, and administrative liability for employee-perpetrated abuse or neglect of residents.

Prohibits the Commissioner from approving applications to acquire another nursing home for five years if the applicant has had three or more civil penalties in the preceding two years, certain Medicare or Medicaid sanctions, or provider agreement terminations or nonrenewals, except for good cause shown.

Requires the Department of Public Health to provide the liability notice to all existing nursing home licensees by January 1, 2014.

Clarifies that the required notice statement does not expand or affect existing liability of nursing home licensees and owners under law.
