Allows health care providers to use a National Provider Identifier assigned by the Centers for Medicare and Medicaid Services in place of a federal tax identification number on HCFA1500 health insurance claim forms and successor forms effective October 1, 2013.

Specifies 27 required data elements on HCFA1500 claim forms that, when completed and received by an insurer, cannot result in the claim being deemed deficient for processing purposes, including patient identification, diagnosis codes, dates of service, and provider billing information.

Requires the Commissioner of Public Health to develop a uniform treatment authorization form for mental health services by January 1, 2014, in consultation with the Insurance Commissioner, Commissioner of Mental Health and Addiction Services, Attorney General, and Healthcare Advocate.

Mandates that the uniform mental health authorization form comply with state and federal privacy laws including HIPAA and state and federal mental health parity laws.

Requires the Commissioner of Public Health to submit any proposed implementing legislation to the joint standing committee with cognizance of public health matters by February 12, 2014.
