Establishes a "Livable Communities" initiative under the Commission on Aging, effective July 1, 2013, to promote best practices and resources for designing communities that allow residents of all ages to age in place.

Defines "livable community" as a community with affordable housing, infrastructure, community services, and transportation options for residents of all ages.

Requires the Commission on Aging to establish partnerships with municipal leaders, planning boards, philanthropic organizations, and health/social services representatives to plan forums, investigate innovative approaches, and identify funding sources for livable communities.

Mandates the Commission on Aging create a single portal on its website by January 1, 2014, providing information and resources about the livable communities initiative.

Requires the Commission on Aging to submit annual reports on the initiative to joint standing committees of the General Assembly beginning July 1, 2014.
