Prohibits police officers from requesting identification or documentation other than operator's license, vehicle registration, or insurance card during traffic stops, and prohibits inquiring about immigration status unless there is reasonable suspicion of a criminal offense by a passenger.

Requires municipal police departments and the Department of Emergency Services and Public Protection to adopt written policies prohibiting stops, detentions, or searches solely based on race, color, ethnicity, age, gender, or sexual orientation.

Mandates development of a standardized form by July 1, 2013 for recording traffic stop information including date, time, location, officer badge number, operator demographics, alleged violation, and disposition; requires duplicate copies be provided to persons stopped.

Requires police departments to provide complaints to the Chief State's Attorney, Office of Policy and Management, and Commission on Human Rights and Opportunities, with annual reporting of traffic stop data.

Establishes the Commission on Human Rights and Opportunities to review traffic stop complaints, determine noncompliance, and prescribe corrective procedures for municipalities or agencies found in substantial noncompliance; effective October 1, 2013.
