Allows adult adopted persons age 21 and older whose adoptions were finalized on or after July 1, 2014 to obtain uncertified copies of their original sealed birth certificates from the Department of Public Health without requiring consent from birth parents.

Establishes a contact preference form system allowing birth parents to indicate whether they want contact with the adopted person, want contact only through an intermediary, or do not want contact.

Requires the Department of Children and Families to maintain contact preference forms and health history forms filed by birth parents and publish statistics on these forms at least twice annually.

Notifies eligible adopted persons that contact preference forms and health history information from birth parents may be available from the Department of Children and Families when they receive their original birth certificate.

All provisions take effect July 1, 2014 and apply only to adoptions finalized on or after that date.
