Commissioner of Public Health must develop a plan to reduce chronic disease incidence (cardiovascular disease, cancer, stroke, lung disease, metabolic disease) and improve chronic care coordination in the state, in consultation with the Comptroller and representatives of hospitals, health care facilities, and local/regional health departments.

Commissioner must submit an annual report by January 15th to the joint standing committee on public health detailing chronic disease burden, funding sources, implementation progress, and care coordination efforts.

Annual reports must include assessment of financial effects of chronic diseases on the state and healthcare facilities, identification of most prevalent conditions, and detailed recommendations for hospitals to reduce readmission rates and improve transitional care and drug therapy monitoring.

Commissioner must post reports on the Department of Public Health website within 30 days of submission to the General Assembly.

Effective from passage upon enactment.
