Prohibits altering, amending, or issuing new certificates unless they accurately reflect the referenced insurance policy's provisions.

Prohibits certificates from warranting that the referenced insurance policy complies with contract requirements, even if a contract number appears on the certificate.

Prohibits requesting opinion letters or other documents inconsistent with these provisions, though insurers may prepare addendums that clarify coverage; the commissioner may investigate violations.
