Prohibits health insurers and related entities from including contract provisions requiring optometrists to accept insurer-set payment amounts for vision services or procedures not covered under a patient's plan, effective January 1, 2015.

Requires optometrists to charge no more for non-covered services than their usual and customary rates, matching existing rules that apply to dentists.

Mandates vision plan evidence of coverage documents include a notice informing patients that non-covered optometric services may not be offered at discounted rates and recommending advance treatment plans with cost estimates.

Requires optometrists to post visible notices that services not covered under insurance policies might not be offered at discounted rates.

Exempts self-insured vision plans and contracts derived from collective bargaining agreements from these requirements.
