Requires the Commissioner of Public Health to develop and implement a plan for reducing chronic disease incidence and improving care coordination, consistent with the Healthy Connecticut 2020 plan and the state healthcare innovation plan.

Directs the plan to address chronic cardiovascular disease, cancer, lupus, stroke, chronic lung disease, diabetes, arthritis, and behavioral health disorders.

Mandates biennial reports to the joint standing committee on public health starting January 15, 2015, posted on the Department of Public Health website within 30 days of submission.

Reports must include descriptions of prevalent chronic diseases, affected populations, financial impacts, existing programs, funding sources, care coordination efforts, and recommendations for reducing chronic disease incidence.

Effective date: October 1, 2014.
