Authorizes State Bond Commission to issue up to $5 million in bonds to fund a Department of Rehabilitation Services grant program providing home modifications and assistive technology devices for eligible persons age 65+ and adults with disabilities to remain in their homes.

Establishes eligibility criteria for grant recipients as persons at risk of losing independent living ability without modifications or devices, with household income not exceeding 400% of federal poverty level.

Creates a matching grant program through the Commission on Aging providing up to $100,000 per grant to municipalities and nonprofits for aging-in-place initiatives including affordable housing, community services, zoning changes for accessory apartments, and transportation infrastructure.

Appropriates $1 million from the General Fund to the Commission on Aging for fiscal year 2015 to fund the matching grant program, with recipients required to provide 50% matching funds.

Requires Commission on Aging to recognize livable communities by January 1, 2015, and directs Department on Aging and Department of Social Services to hold quarterly meetings with nutrition service stakeholders to improve elderly nutrition program administration.
